Attention Athletes
To participate in school athletic tryouts, training, practices, camps, or games, you must have had a physical evaluation performed within the past 365 days. It is a good idea to have the physical during the summer months so it is active for the entire school year. The FHSAA Preparticipation Physical Evaluation form, along with the other required athletics forms, may be downloaded and printed from our Printable Documents page. The athletics physical exam may be performed by the physician of your choice. |
